Job summary

SCAN Health Plan seeks a Medical Director to provide clinical leadership across the health care continuum, overseeing teams of nurse practitioners and physicians in field-based Oregon operations.

The role requires an MD with an unrestricted license and at least 5 years in geriatrics/palliative care, including home-based or telehealth experience. The position emphasizes quality metrics, education, and strategic collaboration to improve patient outcomes.

Qualifications

• Doctoral Degree M.D. required.
• Unrestricted State Medical License required.
• Geriatrics/Palliative care experience 5+ years of related experience as a physician, preferably with home-based visits, geriatrics, chronic disease management, and hospice and/or palliative care.
• Experience with virtual care/telehealth preferred.
• Knowledge of the treatment of complex chronic diseases, such as dementia, diabetes, heart failure, and COPD.

Responsibilities

• Directly manage a multi-disciplinary team of nurse practitioners and physicians to ensure coordinated and high quality care delivery.
• Track and analyze clinical performance metrics related to health outcomes and utilization rates.
• Use this data to identify areas for improvement and implement strategies to enhance overall care quality.
• Participate in community stakeholder meetings and address any issues related to care provided to their residents.
• Provide education and guidance to other clinical team members related to geriatrics, complex care, and palliative care.
• Actively participate in provider meetings and quality improvement initiatives aimed at improving patient outcomes.
• Work cross-functionally with other leaders to design, lead, and implement strategic initiatives to improve clinical, financial, and operational performance.
