Scan-Health-Plan seeks a Medical Director to provide clinical leadership for home-based geriatric care. You will manage a multi-disciplinary team, analyze outcomes, and guide care quality across a remote/field model.

The role emphasizes aging, chronic disease management, and palliative considerations. Responsibilities include leading rounds, supporting after-hours on-call, maintaining EMR accuracy, and ensuring CMS/HIPAA compliance while collaborating with stakeholders to improve patient

Qualifications

• Doctoral Degree M.D. required.
• Unrestricted State Medical License required.
• Geriatrics/palliative care experience (5+ years).
• Experience with virtual care/telehealth preferred.
• Knowledge of HCC and ICD-10 coding and CMS guidelines.
• HIPAA compliance knowledge and data privacy focus.

Responsibilities

• Lead a multi-disciplinary team of nurse practitioners and physicians to ensure coordinated, high-quality care.
• Track clinical performance metrics and drive ongoing quality improvements.
• Participate in community stakeholder meetings and address care-related issues.
• Provide education on geriatrics, complex and palliative care to team members.
• Conduct in-person and virtual patient rounds; participate in after-hours on-call.
• Maintain EMRs and ensure regulatory compliance and documentation standards.
