Job Description

As a Healthguide, you will be supported by clinical teams in your efforts to strengthen the connection between patients and their primary care physicians or medical practice staff, as well as improve the patients' health and well-being. The Healthguide engages patients, helps them navigate their care, resolve healthcare issues, and enhances communication with their medical practice.

What You’ll Be Doing

• Working primarily remotely with the patient’s medical practice staff and Guidehealth’s Case Management team to manage a targeted patient population.
• Functioning independently and collaboratively as an extension of the medical practice.
• Assisting patients in achieving better health outcomes by addressing questions, connecting them with the practice, implementing care plans, closing care gaps, escalating issues, and navigating resources, including referrals to specialists and community organizations.
• Accurately documenting medical history and issues in the electronic health record (EHR).
• Understanding referral and prior authorization processes.
• Building trusting professional relationships with medical practices and within the organization.
• Meeting Key Performance Indicators to improve patient outcomes and reduce medical costs.
• Engaging high-risk patients via calls, texts, portals, and emails.
• Escalating medical issues appropriately to nurses, physicians, or leadership.
• Adhering to security policies to protect PHI and intellectual property.
• Position may require travel (10-15%) within the Atlanta, GA area for patient and provider engagement, including retinal eye exams with paid training.
