Job Description
As a Healthguide, you will be supported by clinical teams in strengthening the connection between the patient and their primary care physician/medical practice staff, as well as improving the patient’s health and well-being. The Healthguide engages patients, helps them navigate their care, solve healthcare issues, and improves communication with their medical practice.
What You’ll Be Doing
- Working primarily remotely with the patient’s medical practice staff and Guidehealth’s Case Management team to manage a targeted population of patients.
- Functioning independently and collaborating as an extension of medical practice.
- Assisting patients in achieving better health outcomes by addressing questions, connecting them with the medical practice, implementing care plans, closing care gaps, escalating issues, and navigating resources including referrals to specialty care and CBOs.
- Documenting patients’ medical history and issues accurately in the electronic health record (EHR).
- Understanding the referral and prior authorization process.
- Building trusting, professional relationships with medical practices and within Guidehealth.
- Meeting Key Performance Indicators to improve patient outcomes and reduce medical costs.
- Using effective communication methods (calls, texts, portals, emails) to engage high-risk patients.
- Escalating medical issues appropriately to nurses, physicians, or leadership.
- Following all security policies to protect PHI and intellectual property.
- Position may require travel (10-15%) within Atlanta, GA for patient and provider engagement, with paid training for retinal eye exams.
