Job summary

MDS Solutions, a division of Key Rehabilitation, is seeking a Remote MDS Coordinator to work with contract partners to plan, organize, and coordinate MDS completion following current regulations. RN licensure is required; LPN does not meet minimum qualifications.

Qualifications

• RN license is mandatory to qualify.
• Experience coordinating MDS with an interdisciplinary team.
• Knowledge of RAI guidelines and PDPM.
• Ability to use EMR and basic computer skills.

Responsibilities

• Coordinate MDS scheduling and ensure timeliness per RAI guidelines.
• Provide Medicare/Medicaid oversight to ensure appropriate services and reimbursement.
• Develop individualized resident care plans with the interdiscliplinary team.
• Review and verify MDS documentation and charting requirements.
• Ensure timely submission of all MDS data to the state database and follow up on actions.
• Promote quality care through QI/QM data with facility team and identify trends.
