Job summary

MDS Solutions, a division of Key Rehabilitation, is seeking a Remote MDS Coordinator to plan, organize, and coordinate the completion of the Minimum Data Set (MDS) in accordance with current Federal and State Regulations. This role is remote and collaborative.

RN license required; LPN will not meet the minimum qualifications. The position offers flexible, creative working environment, benefits, CE opportunities, and advancement opportunities within a supportive team.

Qualifications

• RN license required; LPN not sufficient.
• Minimum 3+ years of nursing experience in MDS assessment.
• Thorough understanding of PDPM requirements.
• Ability to navigate EMR systems with accuracy.
• Strong communication and teamwork with interdisciplinary staff.

Responsibilities

• MDS scheduling and timely submission to the state database per RAI guidelines.
• Coordinate care plan development and completion with the interdisciplinary team.
• Provide Medicare/Medicaid case mix oversight and ensure appropriate reimbursement.
• Review and update care plans quarterly to reflect resident status and goals.
