Qualifications

• The candidate holds a high school diploma or equivalent; a Bachelor's degree is preferred.
• Community Health Worker certification is preferred and must be obtained within 12 months of hire.
• Experience in community outreach or healthcare settings is advantageous.

Responsibilities

• Build trust with individuals and communities to connect them with health and social services.
• Identify barriers such as housing, food insecurity, and transportation; link to resources.
• Conduct outreach, home visits, and participate in health fairs to engage underserved populations.
• Maintain accurate records to support advocacy and program evaluation.
• Assist with health education and basic screenings under supervision.
• Support ongoing community health programs.
