Job summary

Aperion Care remote position handles triage of daily referral requests, ensuring complete information before passing to case managers for skilled care stays across plans. Verifications of benefits for commercial and Medicare Advantage plans are included to support referrals.

The role acts as a liaison among marketing, admissions and case management, ensuring efficient, compliant referral workflows and comprehensive data handling.

Qualifications

• College degree preferred.
• Experience in a skilled nursing facility.
• Understanding of how claims are paid under Commercial and Medicare Advantage plans for skilled services.
• 2 years working with MCO insurance experience preferred.
• Self-starter with results orientation.
• Strong organizational, planning and problem-solving skills.
• Able to work multiple tasks concurrently.
• Understanding of insurance types at a skilled nursing facility.
• Knowledge of Medicare rules as they apply to SNF.

Responsibilities

• Primary liaison between marketing liaisons, facilities admissions and MCO Case Management for new referrals and readmissions.
• Communicate with marketing teams, facility contacts, and case management about referral process.
• Review referral requests for completeness and readiness for case manager.
• Perform benefit verifications on all referrals per processes.
• Hold facilities accountable to the referral process for MCO cases.
• Maintain/Close out Manage Care Referral Log.
• Assist Case Management Consultant – obtaining authorization, communications with communities and insurers.
