Job summary

Dignity Health MSO is hiring a Quality Liaison LVN to educate providers and clinics on clinical quality measures and documentation. Travel 50–75% of the workweek to provider sites and clinics across CA is required.

The role supports data collection, trend analysis, and action planning with providers to close gaps in care. Remote work is provided with equipment, and CA LVN licensure is required.

Qualifications

• Two years of HEDIS data collection and measures improvement with analytical review and/or Quality Improvement.
• Experience in data and chart reviews to educate providers and staff.
• Current CA LVN license.
• Strong written and verbal communication, judgment, problem solving, and presentation skills.

Responsibilities

• Advise and educate providers and staff in clinical quality measures, medical record documentation, and coding guidelines. Collects, summarizes and trends provider performance data to identify opportunities for provider improvement and closing gaps-in-care.
• Reports provider/clinic specific metrics and identifies needs and barriers to quality improvement; develops action plans with providers.
• Performs medical record review or audits and documents encounters.
• Partners with physicians and staff to encourage member clinical participation in wellness and education; provides resources on clinical quality measures.
• Update data and maintain data integrity in population health platforms and EHR systems.
