Job summary

Blue Zones Health is seeking an LVN for Utilization Management & Inpatient Case Management in a fully remote role. The ideal candidate will perform concurrent reviews, monitor inpatient utilization, and coordinate transitions of care while collaborating with hospital teams and health plans.

Requirements include CA LVN license, 3+ years nursing experience, and knowledge of CMS/Medicare Advantage processes. Remote work flexibility is supported with strong clinical judgment and communication

Qualifications

• Graduate of an accredited LVN program.
• Current, active California LVN license.
• Current BLS certification preferred.
• Minimum of 3 years clinical nursing experience.
• Minimum of 1-2 years in utilization management, inpatient case management, concurrent review, or managed care.
• Experience with Medicare Advantage, IPAs, health plans.
• Experience with inpatient authorization processes, hospital levels of care, discharge planning.
• Remote work experience preferred.

Responsibilities

• Weekend & After-Hours On-Call Coverage.
