Job summary

Blue Zones Health seeks a Licensed Vocational Nurse (LVN) for Utilization Management & Inpatient Case Management in a fully remote role based in California. You will monitor inpatient services, perform concurrent reviews, and coordinate transitions of care while collaborating with hospital teams and health plans.

The ideal candidate has strong managed care knowledge, California LVN licensure, and 3+ years of nursing with 1–2 years in utilization management. Remote work flexibility is required.

Qualifications

• Graduate of a vocational nursing program.
• Current, active, and unrestricted California LVN license.
• Minimum 3 years of clinical nursing experience.
• 1–2 years of utilization management or inpatient case management experience.
• Experience with Medicare Advantage, IPA health plans, and managed care.

Responsibilities

• Perform inpatient utilization management and concurrent reviews.
• Coordinate inpatient case management and discharge planning.
• Support outpatient utilization management activities as needed.
• Maintain documentation in the utilization management system.
• Provide cross-coverage for UR staff during peak times.
