Qualifications

• Active, unrestricted California LVN/LPN license in good standing.
• Minimum of two years in care coordination, chronic care management, clinical nursing, or related setting.
• Ability to work independently in a remote environment following clinical protocols.
• Experience with CCM, TCM, or RPM.
• Experience in telehealth, population health or virtual care.
• Strong patient education and care coordination skills.
• Sound clinical judgment within LVN/LPN scope.
• Comfort using EHRs and virtual care platforms.

Responsibilities

• Support chronic disease management and care coordination for California patients.
• Assist with patient assessments and develop individualized care plans.
• Coordinate follow-ups and care transitions with multidisciplinary teams.
• Engage patients with education on medications and self-management.
• Identify care gaps and escalate to the appropriate clinician.
• Ensure privacy, security, and telehealth compliance in daily work.
