PharmD Live is seeking a Licensed Vocational Nurse (LVN) to support California patients through virtual care management and coordination services. The role focuses on chronic disease management, patient education, and continuity of care within a multidisciplinary clinical team.

The LVN will work fully remote, collaborating with supervising clinicians, manage care plans, coordinate follow-ups, and ensure compliant documentation and privacy standards in telehealth settings.

• Active, unrestricted California LVN/LPN license in good standing.
• Minimum of two years of experience in care coordination, chronic care management, clinical nursing, or a related setting.
• Strong communication, documentation, and patient engagement skills.
