Job summary

Community Health Network is seeking a Discharge Care Manager LPN to support IHCI initiatives. You will coordinate post-discharge care, work with patients and families, and collaborate with physicians and the population health team to improve outcomes.

You will conduct assessments, plan and monitor care, and provide education and self-management support tailored to each patient. This role emphasizes telephonic and occasional in-person contact within the Evansville area.

Qualifications

• 2 year / associate degree in practical nursing.
• Licensed as an LPN with a valid license to practice in Indiana (NLC).
• 3+ years of experience in home health, ambulatory care, community public health, case management, or coordinating care across settings.

Responsibilities

• Discharge Care Manager LPN connects with patients by phone or in person in physician offices or hospitals.
• Conduct patient surveys to assess medical, behavioral, pharmaceutical, and social needs and identify gaps in care.
• Manage a care plan with the patient, physician and population health team to address needs and improve health outcomes.
