Job summary

Community Health Network in Indiana is seeking a Care Transition Liaison RN to coordinate value-based discharge planning, provide disease-specific education, and ensure patients have a safe and timely transition from hospital to home.

Based onsite at Deaconess Midtown Hospital in Jacobsville, IN, you will partner with IHCI and inpatient teams to arrange referrals, connect patients with a primary care physician and community resources, and advocate for the patient throughout the care continuum.

Qualifications

• 2 year / associate degree in nursing (required).
• Licensed as a Registered Nurse (RN) in Indiana (required).
• Preferred Certifications: CCM or ACM.
• 5+ years nursing experience, preferably inpatient.

Responsibilities

• Coordinate value-based discharge planning and ensure education is provided.
• Connect patients with a primary care physician and community resources.
• Advocate for patients and work with IHCI and hospital teams to ensure seamless transitions.
