Job summary

Community Health Network is seeking a Care Transition Liaison RN to work onsite at Deaconess Midtown Hospital in Evansville, IN. The role focuses on smooth care transitions for value-based patients discharged from the hospital, coordinating with IHCI and inpatient teams.

Responsibilities include ensuring disease-specific education, connecting patients to a primary care physician, and coordinating discharge planning and community resources for a safe home transition.

Qualifications

• 2 year / associate degree in nursing (required).
• Licensed as a Registered Nurse (RN) with a valid license to practice in Indiana (NLC) (required).
• Preferred Certifications/Licensures: CCM or ACM or equivalent certification.
• 5+ years: nursing experience required, preferably in an inpatient setting.

Responsibilities

• Ensure value-based patients discharged from the hospital have disease-specific education and are connected to a primary care physician.
• Coordinate discharge planning with inpatient case management teams.
• Serve as patient advocate to create seamless transitions for discharge.
• Coordinate consultations, referrals and community resources to support the patient upon discharge.
