Job summary

AccentCare in Chicago area is seeking a dedicated LVN/LPN to provide direct patient care under RN supervision, serving pediatric through geriatric patients. This full-time role covers Chicago (S) to Hyde Park, with a schedule of M-F 8-5 and a pay range of $66,000–$77,000 based on experience.

You will perform nursing visits per plan of care, document progress, and coordinate with the home health team. A 1-year LVN/LPN experience and active state license are required.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Licensed as LVN/LPN in the state of operation.
• One year of LVN/LPN work experience.
• Ability to provide nursing visits per treatment plan and maintain medical records.
• Valid CPR certification required.
• Licensed driver who can travel to all locations.

Responsibilities

• Performs technical nursing functions and direct patient care under the supervision of an RN.
• Promotes and maintains optimal client care under the supervision of an RN.
• Maintains a safe patient environment.
• Identifies and communicates significant abnormalities in patients data to case manager and other home health team members.
• Administers prescribed medications and treatments as permitted by law. Assesses medicines for interactions and adverse effects and reports problems to the physician.
• Re-evaluates client's nursing needs and reports findings to the Case Manager.
• Counsels and instructs the client and family in meeting nursing needs.
• Prepares clinical and progress notes.
• Submits all documentation in a timely manner per agency policy.
• Documentation on patients chart correlates with nursing care plan.
• Communicates frequently and works closely with other home care team members.
• Assists in coordinating services provided to client
