The LVN/LPN must travel to all business locations and maintain complete medical records. Requirements include graduation from an accredited LVN/LPN program, active state licensure, CPR certification, and at least one year of LVN/LPN experience.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) in the state of operation.
• At least one year of LVN/LPN experience.
• CPR certification and valid driver’s license required.
• Ability to travel to all business locations and provide nursing visits.

Responsibilities

• Performs technical nursing functions and direct patient care under RN supervision.
• Promotes optimal client care under RN supervision.
• Maintains a safe patient environment.
• Administers prescribed medications and treatments; assess medicines for interactions and adverse reactions.
• Re-evaluates client’s nursing needs and reports findings to Case Manager.
• Counsels and instructs client and family in nursing needs.
• Prepares clinical and progress notes.
• Submits documentation timely per agency policy.
• Coordinates services with home care team.
