Horizon Blue Cross Blue Shield of New Jersey is seeking a Clinical Care Associate - LPN to provide field-based support for Care Managers. You will help coordinate services, locate providers, schedule appointments, and assist with authorizations and denials as appropriate, under the supervision of the Care Manager.

The role emphasizes member interaction, education, and documentation to ensure quality, cost-effective care aligned with plan guidelines. NJ residency is required by policy.

Formación

• High School Diploma/GED required.
• Prefers healthcare industry experience.
• Prefers minimum of one year experience in home care, discharge planning, or case management; elderly frail population preferred.
• Prefers minimum one year experience in the managed care industry.

Responsabilidades

• Assist with scheduling and coordination of service needs identified in the member's Plan of Care.
• Assist members in locating health care professionals and coordinating services and authorizations.
• Ensure timely member follow up and effective telephonic communication.
• Provide education and information to support member health and well-being.
• Document accurately and comprehensively per standards and policies.
