Job summary

CareHarmony is seeking a Licensed Practical Nurse (LPN) to collaborate with providers in a remote, value-based care management role. You will manage chronic care needs, coordinate resources, educate patients, and ensure timely service delivery across Home Health and related services.

Qualifications

• Active NY LPN license required.
• At least 3 years of direct patient-facing work experience.
• Experience identifying resources and coordinating needs for chronic care management patients welcome.

Responsibilities

• Identify and coordinate community resources for patients.
• Provide patient education and health literacy on chronic condition management.
