Job summary

UChicago Medicine in Chicago, IL seeks a Transplant Nurse Navigator to coordinate care for liver transplant patients. This hybrid role offers work-from-home flexibility with on-site visits as needed, based in the greater Chicagoland area.

You will liaise with the Liver transplant team, assess patient needs, provide education, monitor symptoms, and support discharge planning to prevent readmission. Licensure as an Illinois RN and 2+ years of clinical nursing experience are required.

Qualifications

• Active Illinois RN license.
• Minimum two years of clinical nursing experience.
• Strong interpersonal and communication skills.
• Ability to work remotely with onsite requirements.

Responsibilities

• Provide care coordination and patient education in the service line.
• Support discharge planning for the service line.
• Address care gaps by contacting patients and handling calls/messages.
• Liaise with patients, families, and multidisciplinary teams.
• Educate patients and families on disease, treatments, side effects, and warning signs.
• Review clinical documentation to improve length of stay.
• Develop patient education materials, pathways, and quality studies.
• Serve as primary contact for discharged patients to prevent readmission.
