Northwestern Medicine is seeking a Pre-Heart Transplant Nurse Coordinator (RN) to join the Bluhm Cardiovascular Institute in a full-time day shift in Chicago. The role requires an Illinois RN, BSN, BLS, and 2–3 years of related clinical experience to coordinate transplant care.

The nurse will work under transplant physicians, coordinate care across phases, and liaise with external clinicians and community resources to ensure compliant, patient-centered service.

Qualifications

• BSN required.
• Licensed Registered Nurse (RN) in the state of Illinois.
• CPR-Basic Life Support (BLS).
• Minimum 2-3 years of recent & related clinical experience.

Responsibilities

• Plan, direct, provide, and document appropriate care for patients in the designated care setting.
• Coordinate comprehensive care including consent, data collection, follow-up visits, referrals, testing, treatment, and long-term follow-up.
• Collaborate with the interdisciplinary team to maximize wellness and patient outcomes.
• Develop and provide education for patients, family, caregivers, and staff.
• Serve as a communication link between patient, family, team, and external entities.
• Participate in committees or quality initiatives and ensure compliance with policies and regulatory standards.
• Other duties as assigned.
