Job summary

ForzaCare is seeking a full-time Life Care Planner & Medical Cost Projection Specialist. This predominantly remote role coordinates MSP compliance, MSAs, conditional payments, and cost projections while ensuring CMS guideline adherence.

Limited travel may be required, with responsibilities spanning intake to case completion. The position requires Life Care Planner certification, LCP experience, and relevant licenses/certifications such as RN, CCM, CRC, or CCM.

Qualifications

• Active RN license or CRC/CCM credentials with a related degree required.
• Life Care Planner certification and prior LCP experience required.
• CCM, CRC or RN with LCP background preferred depending on role.
• CMSP certification is a plus and other related certifications are preferred.
• Valid driver's license and reliable transportation are required.

Responsibilities

• Oversee workflow from intake to completion, ensuring timelines and service standards are met.
• Ensure all work aligns with CMS guidelines and MSP regulations, and manage CMS submissions.
• Assist with Medicare Set-Asides, conditional payments, and cost projections.
• Perform quality checks on reports and supporting documentation for accuracy.
• Communicate case status and requirements with clients and internal teams.
