Job summary

Concert Health in the United States (WA) is hiring a Licensed Clinician (LICSW/LMHC) in a remote capacity. The role supports pediatric and adult patients within the Collaborative Care model and reports to Clinical Excellence leadership.

You’ll provide evidence-based interventions, coordinate with PCPs, and participate in supervision and care management activities. Qualifications include a WA license, a master's in social work or counseling, and 1–5 years of licensed practice.

Qualifications

• Master’s Degree in Social Work, Counseling, or related field from an accredited program.
• Licensed to practice in the state of Washington as an associate clinician under supervision OR as an independent clinician.
• Minimum 1–5 years experience as a licensed professional.
• A secure, high-speed internet connection capable of supporting video conferencing.

Responsibilities

• Provides care to patients based on needs of assigned Care Pod and customizes treatment plans.
• Coordinates behavioral health care with primary care providers and other services.
• Screens patients for behavioral health and substance use disorders using standard tools.
• Monitors symptoms and treatment side effects with goal of symptom reduction.
• Supports medication management and adherence under PCP guidance.
• Tracks outcomes in a registry and documents encounters for sharing with care team.
• Educates patients about behavioral health options and plans mutually with patients/families.
