Concert Health is hiring a Collaborative Care Clinician I in a fully remote role, serving pediatric and adult patients. You will deliver evidence-based interventions through the Collaborative Care Model, tailored to each patient’s needs, under supervision.

The position emphasizes coordination with primary care teams and ongoing documentation in our EHR and Salesforce systems. The role requires a Master’s in Social Work or Counseling and licensure in Washington state (ASW/APCC under supervision

Qualifications

• Master’s Degree in Social Work, Counseling, or related field.
• WA state licensure either ASW/APCC under supervision or independent clinician.
• 1–5 years licensed professional experience.
• Reliable high-speed internet for video sessions.

Responsibilities

• Provide care to patients within the Care Pod and tailor treatment plans.
• Coordinate behavioral health care with primary care providers.
• Screen patients using PHQ9, GAD7, and other tools.
• Monitor symptoms and treatment side effects to reduce symptoms.
• Support medication management and track outcomes in the registry.
• Document progress in Salesforce and EHR for sharing with clinicians.
