Job summary

ConcertoCare is seeking a Social Worker (LCSW) to join an interdisciplinary team delivering integrated medical, behavioral, and social care to patients with complex needs. The role involves virtual and telephonic visits, comprehensive assessments, and coordinated treatment planning within a value-based care model.

The ideal candidate has an MSW, current WA or CA licensure, and 2+ years of healthcare/behavioral health experience.

• Current California or Washington independent licensure required.
• 2+ years of healthcare or behavioral health experience.
• Geriatric care experience is highly desired.
• Ability to manage patient complexity and diverse needs.

Responsibilities

• Supports the behavioral health needs of patients on an assigned patient caseload in accordance with established clinical practices.
• Uses evidence-based models of care to deliver brief treatment for common mental health issues such as depression, anxiety, coping with loss, mood management, and substance-use disorders.
• Support psychotropic medication management by collaborating with prescribing providers and coordinating with care teams on urgent psychotropic issues.
• Participate in caseload reviews and clinical team meetings; communicate patient behavioral health needs effectively within the team.
• Document patient encounters and referral activities in the electronic health record, ensuring notes are secure per policy.
• Demonstrate subject matter expertise in behavioral health and share knowledge with the team.
• Ensure care respects diverse values, beliefs, and linguistic needs.

The ConcertoCare Social Worker - LCSW is a core member of the interdisciplinary team responsible for providing integrated medical, behavioral, and social care to patients with complex healthcare needs.

Our Social Workers provide clinical care to medically and socially complex patients who live in community-based settings, including assisted living facilities, adult family homes, and private homes, through virtual and telephonic visits. This position ensures that the patient's biopsychosocial needs are adequately met through the completion of comprehensive behavioral health assessments and engaging the patient, caregiver(s), and care team in treatment planning and care coordination activities. The Social Worker is responsible for the provision of brief evidence-based behavioral health therapies and crisis intervention as a behavioral health expert within an integrated health care model. Social Workers also work collaboratively with other members of the local care team and is key in ensuring access to equitable behavioral health care in alignment with the organization's mission, vision, and values.

ESSENTIAL DUTIES AND RESPONSIBILITIES

• Supports the behavioral health needs of patients on an assigned patient caseload in accordance with the established clinical practices of the behavioral health team
• Uses evidence-based models of care to deliver brief treatment for common mental health issues such as depression, anxiety, coping with loss, mood management, and substance-use disorders
• Support psychotropic medication management by collaborating with prescribing providers, supporting treatment adherence needs, partnering with patients/caregivers for continuous monitoring of efficacy and potential negative side effects, and coordinating with internal psychiatrists and clinical care teams to address urgent psychotropic issues
• Participate in regularly scheduled caseload reviews and clinical team meetings; facilitate effective communication regarding patient's behavioral health needs among all members of the clinical team both verbally and in writing
• Complete and document assessment of patients in a timely manner
• Facilitate treatment plan changes for patients who are not demonstrating improvement in symptoms by consulting with patient's primary care provider/members of the clinical team and using measurement-based care to attune behavioral health interventions to the existing needs and symptom burden of patients
• Demonstrate the ability to serve as a subject matter expert in a particular area(s) of behavioral health care and supports the behavioral health team at large by sharing knowledge and resources related to area of expertise
• Ensure care is delivered in a manner that honors patient's diverse values, beliefs, behaviors, backgrounds, social, cultural, and linguistic needs
• Document patient encounters, referral activities, and other pertinent information in electronic health record ensuring all encounter notes are locked in accordance with company policy.
