Job summary

Qualifications

• Must be independently licensed (LICSW, LMFT, or LPC) and provide Diagnostic Assessments, Individual Therapy, and Service Plans.
• Master’s Degree in Social Work or comparable Human Services field.
• 2 years of direct service with youth and families preferred.
• Valid driver’s license and reliable transportation.
• Ability to work flexible and non-traditional hours.
• Respectful and professional with diverse cultures/backgrounds.
• Proficient using computers and common office software.

Responsibilities

• Provide individual, group, and family mental health services; case management and transportation as needed.
• Coordinate and monitor services for youth and families across mental health, medical, educational, psychological, vocational, and social sectors.
• Travel daily to deliver community-based services in compliance with contracts and regulations.
• Complete all required documentation promptly.
• Engage in monthly/quarterly CQI activities and weekly supervision.
