Overview
CareHarmony's Care Coordinators (LPN/LVN) work comprehensively with providers to deliver value-based care management initiatives for their patients. CareHarmony is seeking an experienced Licensed Practical Nurse to work 100% remote with at least 3 years of direct patient-facing work experience. The role thrives in a fast-paced environment, requires self-motivation and impeccable attention to detail, and focuses on the impact on a patient’s healthcare journey. You will have experience identifying resources and coordinating needs for chronic care management patients.

Responsibilities
- Manage patient census with a resolution-driven approach to close gaps in clinical and non-clinical patient care.
- Identify and coordinate community resources with patients that would benefit their care.
- Provide patient education and health literacy on the management of chronic conditions.
- Perform medication management, including identifying potential medication concerns, reconciliation, adherence, and coordinating refills.
- Assist in ensuring timely delivery of services to patients (Home Health, DME, Home Infusion, and other critical needs).
- Resolve patients’ questions and create an open dialogue to understand needs.
- Assist/manage referrals and appointment scheduling.
Qualifications
- Active Multi-State/Compact License (LPN/NLC/LVN).
- Technical aptitude – Microsoft Office Suite.
- Excellent written and verbal communication skills.
Pluses
- Epic Experience.
- Bilingual.
- Additional single-state licensures (LPN).

- Sedentary role with prolonged sitting.
- Ability to speak clearly and listen attentively, often by telephone, for extended periods.
- Ability to understand, process, and take thorough notes in real time during telephone conversations.

Pay
- Starting at $21/hour with potential to earn up to $28/hour based on production.
