Qualifications

• Ability to provide therapy independently within a supported model
• Strong clinical judgment for assessment, diagnosis and treatment planning

Responsibilities

• Conduct biopsychosocial assessments to evaluate client needs and risks
• Provide individual, family, and group therapy using evidence-based modalities
• Develop and implement individualized treatment plans with clients and care teams
• Monitor client progress and adjust plans as needed
• Provide crisis intervention and safety planning
• Collaborate with physicians, nurses, case managers and community health workers
• Complete timely clinical documentation in the EHR and maintain HIPAA compliance
• Engage with communities and participate in educational workshops
• Support care coordination and referrals to specialty services
