Job summary

Ascend Healthcare is seeking a Bilingual Licensed Clinical Social Worker (LCSW) for a remote California role. The clinician will provide telemedicine counseling to patients through community health center partners, using evidence-based modalities and comprehensive documentation.

The position requires an active CA license, remote work comfort, and the ability to collaborate with a multidisciplinary team to ensure effective care and recovery.

Qualifications

• Active, unrestricted mental health license in state of primary residence and California
• Comfortable working in a remote environment
• Trained and/or certified in evidence-based trauma modalities such as TF-CBT, EMDR, Brain Spotting, Somatic-Based Approaches, Narrative Therapy, Psychodynamic Attachment-Based Therapy, CPT, Polyvagal Theory, or Family Systems (IFS).

Responsibilities

• Provide counseling and therapy via telemedicine to patients of Community Health Center partners
• Assess and develop treatment plans and document clinical notes in patient records
• Collaborate with Multidisciplinary Team for case planning and continuity of care
• Monitor patient progress and adjust treatment plans as needed
• Support crisis intervention and safety planning for individuals and families
