• Experience in assessment, treatment planning, and documentation in EMR.

Responsibilities

• Conduct comprehensive clinical assessments and develop treatment plans.
• Deliver individual and group therapy to patients.
• Provide case management and care coordination.
• Collaborate with patients, families, and care teams; educate on resources.
• Document all clinical work thoroughly in the EMR; participate in meetings.
