Job summary

Icon Health is seeking a board-certified physician to serve as collaborating physician for our Advanced Practice Provider team. The physician must live and practice in Kentucky and be available by phone or electronic means as APPs provide services.

The role supports licensing, credentialing, CAQH updates, payer enrollment, and clinical collaboration under the Practice Agreement. It is a remote, contract-based opportunity paid per APP, with a Monday–Friday schedule.

Qualifications

• Board certified in Sports Medicine, PM&R, Physiatry, Interventional Pain Medicine, or similar.
• Active license in Kentucky in good standing.
• Physically located and practicing in Kentucky.
• Willing to support credentialing submissions and CAQH profile updates.

Responsibilities

• Serve as collaborating physician for Icon Health APPs practicing in Kentucky.
• Assist with licensing, credentialing, CAQH updates, payer enrollment.
• Be available by telephone or electronic means when APPs provide services.
• Provide clinical collaboration, guidance, and consultation when APPs encounter patient care needs beyond their competency.
• Support appropriate referrals or escalation when clinically indicated.
• Participate in APP competency and qualification evaluation, including peer review.
• Collaborate with Icon Health to ensure APPs have qualified training and experience.
• Review and update the Practice Agreement regularly.
• Sign required collaboration, credentialing, and regulatory documents in a timely manner.
