Job summary

Icon Health is seeking a board-certified Family Medicine physician with Sports Medicine fellowship to serve as a collaborating physician for our APP team in Kentucky. The physician must live and practice in Kentucky and be available by telephone or electronic means to support APPs providing medical services.

The role supports licensing, credentialing, CAQH updates, payer enrollment, and related administrative tasks within Icon Health’s Practice Agreement.

Qualifications

• Board-certified in Family Medicine.
• Fellowship trained in Sports Medicine.
• Active Kentucky license and in good standing.
• Physically located and practicing in Kentucky.
• Willing to collaborate with APPs and support credentialing.

Responsibilities

• Serve as collaborating physician for Icon Health APPs practicing in Kentucky.
• Support licensing, credentialing, CAQH updates, payer enrollment, and related admin tasks.
• Be available by telephone or electronic means when APPs provide medical services.
• Provide clinical collaboration and consultation when APPs encounter more complex needs.
• Assist with referrals or escalation to physicians when clinically indicated.
• Participate in APP competency evaluations and peer review as per standard of care.
• Ensure APPs have required qualifications, training, and experience for authorized medical services.
• Review and update the Practice Agreement regularly and support changes in conditions or regulations.
• Sign required collaboration, credentialing, and regulatory documents timely.
