Job summary

Center for Life Management in New Hampshire seeks an Integrated Care Case Manager to provide care planning, resource identification, and service coordination for adults and elders. The role includes counseling, home and community-based services, and crisis intervention to support overall well-being.

The position is full-time (35 hours/week) with telehealth and community-based work, requiring collaboration with multidisciplinary teams and adherence to program policies.

Qualifications

• Must hold a Bachelor’s Degree.
• Ability to exercise sound clinical judgment in routine, urgent and emergent situations.
• Knowledge of chronic comorbid physical health conditions and CF I services.
• Proficient computer skills and documentation in electronic health records.

Responsibilities

• Provide care planning and monitor progress toward goals.
• Coordinate internal and external supports and services.
• Deliver counseling and Functional Support Services in community or home settings.
• Provide crisis intervention to ensure client safety.
• Document services timely following agency policies.
