Job summary

Gateway Rehab is seeking a home-based Clinical Intake Coordinator to perform pre-admission level of care assessment and case management of patients. This remote role requires residence in the Pittsburgh, PA region, with shifts of 5 days 8 hours or 4 days 10 hours, including some evenings and weekends.

Responsibilities include initial contact, coordinating admissions and transfers, completing ASAM-based assessments, and liaising with UR staff.

Qualifications

Bachelor’s Degree, Master’s Degree preferred.
Experience conducting assessments and evaluations.

Responsibilities

Acts as an initial point of contact to potential patients and referral sources.
Coordinates admissions to inpatient treatment between various locations by communicating with multiple departments.
Completes clinical assessments to help determine level of care based on ASAM criteria.
Uses DSM-5 guidelines for diagnostic criteria.
Coordinates transfers from satellite sites and/or other facilities (hospitals, jails, and other D&A providers).
Schedules outpatient evaluations as needed.
Completes initial inpatient precertification requests with funding sources and ensures all needed documentation is complete.
Acts as a liaison with UR staff and enters authorization information into patient database.
Responds to and provides case management services to referral sources.
