Job summary

Boundless is seeking an Intake Coordinator located in Worthington, OH, to support access to care for individuals served. The role involves intake assessments, referrals, and coordinating with clinical teams using EHR and Microsoft Teams to ensure timely service delivery.

The position is primarily remote, with occasional travel to Boundless locations in Ohio. Candidates should have 3-5 years of customer service experience and a High School Diploma/GED; a Bachelor’s Degree is preferred.

Qualifications

• Strong communication with diverse stakeholders including families, clients, and staff.
• Proficient in using EHR systems and collaboration tools for care coordination.
• Ability to prioritize, plan, and manage multiple workflows independently.

Responsibilities

• Coordinate intake assessments and referrals using professional judgment.
• Manage scheduling and referrals based on clinician skills and patient needs.
• Maintain documentation in the EHR and ensure records compliance.
• Communicate with referral sources, families, and team members via phone, email, and Teams.
• Coordinate with departments to ensure efficient access to care and enrollment.
• Assist with insurance, Medicaid, and enrollment information with Billing.
