I Am Boundless, Inc. in Worthington, OH is seeking a dedicated Case Manager to support individuals with mental health needs through community-based case management and service linkage. This full-time role focuses on coordinating care and outcomes with Boundless staff and outside providers.

Qualifications include a high school diploma, valid Ohio driver's license, and experience with mental health conditions. PSLF-eligible benefits and comprehensive coverage accompany this position.

Qualifications

• Experience working with people with mental health conditions.
• High School Diploma or equivalent.
• Valid Ohio Driver's License with Ohio BMV; proof of auto insurance; insurable driving record (five points or fewer).
• Ability to obtain and maintain First Aid, CPR - adult, child, and infant. (Training will be provided, if not already certified).
