Sanitas in Miami, FL is seeking a Case Management Coordinator – Inpatient to facilitate smooth transitions from inpatient care to the appropriate level of follow-up. You will coordinate discharge planning, arrange post-discharge follow-up, and address barriers to care to reduce readmissions.

The role emphasizes collaboration with hospitalists, payers, and community resources, with a focus on patient-centered planning and effective use of EMR tools to document care plans.

Qualifications

• 3–5 years of experience in clinical, case management, or community health settings.
• Experience in inpatient discharge planning or care coordination.
• Medical Assistant License required.
• Strong communication, counseling, and documentation skills.

Responsabilités

• Initiate discharge planning upon notification of patient admission.
• Engage patients at inpatient or post-acute facilities and assess discharge readiness.
• Collaborate with Hospitalists, discharge planners, and payer care teams to develop discharge plans.
• Coordinate timely follow-up appointments with PCPs and specialists per discharge instructions.
• Educate patients and families about care instructions, resources, and home-based options.
• Document interactions, plans, and care coordination activities in the EMR.
