Job summary

Optum Infusion Pharmacy is seeking a Clinical Coordinator RN for a seasonal assignment. This full-time telework role runs October through March, 40 hours per week, Monday–Friday, 8am–5pm MT.

The coordinator assesses referrals, verifies orders, coordinates nursing care, and uses EMR for documentation while collaborating with a multidisciplinary team. Required are an active Compact RN license and 2+ years in infusion therapy/home health, with strong communication and organizational skills.

Qualifications

• Current, unrestricted Compact RN license.
• 2+ years of infusion therapy and home health experience.
• Knowledge of Microsoft Office and EMR programs.
• Ability to work with an interdisciplinary team to provide comprehensive care and support.

Responsibilities

• Receives, evaluates, and executes initial patient intake to ensure timely nursing services.
• Reviews patient chart for admission criteria and safety of therapy.
• Communicates with physicians, pharmacists, and outside providers to ensure quality and continuity of care.
• Maintains EMR documentation and clear communication of patient status during care coordination.
• Maintains current nursing licensure in all states served.
• Participates in staff meetings and adheres to training requirements.
