Job summary

Palmetto Infusion seeks a Specialty Infusion Care Coordinator (RN) to coordinate home infusion therapy, ensuring seamless care from admission to completion. The role acts as a liaison among home infusion pharmacy, home health, hospitals, physicians, and managed care providers.

Schedule: Mon–Fri, 8:00 AM–5:00 PM; some Saturdays as needed. Service areas include Greenville, SC and surrounding regions.

Qualifications

• Graduate from an accredited school of nursing.
• Active Registered Nurse License in the state(s) of practice.
• BLS or obtain upon hire.

Responsibilities

• Coordinates patient care activities among staff and contracted agency resources to ensure high-quality, efficient delivery of home infusion services.
• Oversees adherence to physician orders, company policies, scope of nursing practice, and regulatory requirements.
• Implements and maintains care plans aligned with patient needs and therapy goals.
• Provides direct patient care as needed to promote optimal outcomes and service levels.
• Monitors performance of internal and external care providers and ensures clinical standards.
• Facilitates timely communication within the interdisciplinary care team and with stakeholders.
• Supports onboarding of new staff and contracted partners; participates in quality initiatives.
