You will handle patient intake, review admission criteria, coordinate care with providers, use EMR systems, maintain licensure in all serviced states, and educate staff and families about Optum Infusion Pharmacy

Qualifications

• Current, unrestricted Compact RN license required.
• 2+ years of infusion therapy and home health experience.
• RN licensure in at least one state; willingness to obtain additional RN licenses.
• Ability to obtain additional RN licenses in specified states.

Responsibilities

• Receive, evaluate, accept, and execute the initial patient intake process.
• Review patient chart for admission criteria and assess needs.
• Assure timely communication with company personnel and outside providers.
• Establish and maintain positive clinical rapport with patients and families.
• Communicate coordination of care with alternate providers and document communication.
• Collaborate with agencies delivering services on behalf of Optum Infusion Pharmacy.
• Utilize Electronic Medical Record (EMR) systems to communicate patient status.
• Maintain current nursing licensure in all states serviced.
• Assist in securing and maintaining necessary licensures and accreditations.
• Comply with State and Federal regulations and standards of practice.
• Participate in staff meetings with nursing and pharmacy.
• Maintain all mandatory education required upon hire and annually.
• Represent the company in clinical interactions as requested.
• Provide education regarding Optum Infusion Pharmacy services.
• Function as a clinical educator and resource.
• Represent self in a professional and ethical manner.
• Participate in cross-training of various clinical functions.
• Perform other duties as requested.

Requirements

Education

• Current, unrestricted Compact RN license required.

Experience

• 2+ years of experience in Infusion Therapy and Home Health.

Licenses & Certifications

• One of the following licenses required:
  • RN (Registered Nurse) - State Licensure required
• Willing and/or able to obtain additional RN licensures in specified states.
