Job summary

Crossing Hurdles is seeking an experienced medical records reviewer to support AI training and data quality initiatives. The role focuses on validating clinical notes against the medical record, including inpatient HPI, PMH, assessments, and plans, and identifying missing or inconsistent information.

The ideal candidate holds an MD or DO with Board certification (Internal Medicine, Pediatrics, or Hospital Medicine) and has strong inpatient workflow knowledge.

Qualifications

• MD or DO board certified in Internal Medicine, Pediatrics, or Hospital Medicine.
• Experience in inpatient/hospitalist settings is required.
• Strong understanding of inpatient documentation and hospital workflows.
• Comfortable reviewing clinical notes and raw medical record data.
• Strong attention to detail and ability to follow clear guidelines consistently.
• Experience with chart review, CDI, utilization review, or medical coding/QA is a plus.

Responsibilities

• Review medical records, including FHIR data and clinical notes for AI training.
• Check whether clinical information and claims are supported by the medical record.
• Review inpatient documentation, including HPI, PMH, assessments, and plans.
• Identify missing, incorrect, or inconsistent information in medical records.
• Share feedback to help improve AI models and data quality.
• Communicate clearly with the project team about your work and findings.
