Job summary

Riverside Hospice in Grants Pass, Oregon, seeks a dedicated Remote Hospice Nurse Practitioner to support patients and families through virtual assessments and care planning. The NP collaborates with the interdisciplinary team and Hospice Medical Director to ensure compassionate end‑of‑life care with compliant documentation and regulatory adherence.

Occasional in-person visits may be required based on patient needs and regulations, with a strong emphasis onHIPAA compliant communication, quality

Qualifications

• Graduate of an accredited Nurse Practitioner program.
• Current NP and RN licensure in the applicable state.
• Minimum of 1 year NP experience.
• Hospice or palliative care experience preferred.
• CPR certification current.

Responsibilities

• Conduct hospice face-to-face assessments and eligibility determinations.
• Develop and implement individualized plans of care with the interdisciplinary team.
• Provide symptom management and end-of-life care recommendations.
• Maintain accurate electronic medical records and documentation.
