Qualifications

• Ability to work with adolescents, parents and stakeholders; strong organization and communications skills.

Responsibilities

• Collaborate with physicians to determine appropriateness of Homebound referrals.
• Represent LCPS Homebound in transition planning meetings for students with complex needs.
• Assist in matching Homebound instructors with students based on geographic/location needs.
• Develop trainings for school personnel on Homebound strategies and remote learning tools.
• Develop trainings for parents, healthcare providers and community stakeholders.
• Recruit Virginia Licensed Homebound Instructors.
