The ideal candidate holds a Master’s degree and a PP license or eligible endorsement, with at least three years in related fields and strong communication and organizational skills.

Qualifications

• Master's degree required or equivalent.
• Holds or eligible for post-graduate professional license or Pupil Personnel license; endorsement in social work, psychology, or related field.
• Three (3) years of successful experience in either or combination of education, social work, nursing, and psychology.

Responsibilities

• Collaborates with community physicians and psychiatrists to determine the appropriateness of Homebound referrals.
• Serves as an LCPS Homebound representative for students with extensive medical and/or complex mental health needs during transition planning meetings.
• Matches Homebound instructors with students based on geographic location and needs.
• Develops trainings for school personnel on Homebound prevention strategies and student transition meetings.
• Develops trainings for school personnel, parents, healthcare providers and community stakeholders on supporting chronically ill Homebound students.
