Job summary

Vitability Health is recruiting a Practical Nurse (LPN) to join its remote patient monitoring program and provide in-home care for patients in the NYC area. You will perform routine nursing tasks, monitor vitals, and document care with precision.

You will educate patients on managing chronic conditions, use motivational communication to drive health improvements, and support longitudinal patient engagement within RPM initiatives.

Qualifications

• Bilingual Spanish; experience with Medicare patients; strong communication skills.
• Technically comfortable with iPads for charting and records.
• Valid driver’s license with clean driving record and ability to travel to homes.

Responsibilities

• Perform routine primary care nursing tasks: vital signs, glucometer tests, and record maintenance.
• Document care consistently with state regulations and RPM program requirements.
• Educate patients using motivational techniques to support health behavior change.
• Deliver in-home care and coordinate with care teams to improve outcomes.

Qualifications

• Comfortable driving to patient’s homes in NYC and providing care
• Comfortable seeing patients independently and delivering a positive experience
