Qualifications

• Bachelor’s degree in nutrition, nursing, kinesiology, health education, community health education, public health, or psychology.

Responsibilities

• Provide telephonic, in-person, and/or virtual health coaching by assessing needs and readiness to change to help clients set personal wellness goals.
• Document coaching encounters in accordance with program guidelines to coordinate care across the continuum.
• Develop and promote evidence-based health education and wellness offerings for internal and external clients and partners.
• Assist in monitoring, evaluation, and improvement of health coaching programs to meet goals.
• Serve as a wellness and health education resource to colleagues, clients, brokers, payers, and community partners.
