Job summary

Elevance Health is seeking a Health Program Representative I to serve as the initial connector between Care Management programs and eligible members. This role offers virtual work with in-person training when required, a Monday–Friday schedule, and the flexibility to engage members across multiple communications channels.

You will gather member information, explain program features, route inquiries, and support clinicians with referrals while maintaining accuracy and empathy.

Qualifications

• Requires HS diploma or the equivalent and a minimum of 6 months experience in a telesales or customer service; or any combination of education and experience which would provide an equivalent background.
• Proficient in computer/keyboarding and familiar with program benefits.
• Knowledge of company products, services and benefits preferred.

Responsibilities

• Handle inbound/outbound enrollment and engagement calls with eligible members.
• Gathers and records member information in accordance with policies and procedures via telephone.
• Encourages participation in Care Management programs by explaining features and value.
• Records call details and uses scripts as needed; directs inquiries to clinical staff.
• Initiates referrals and manages consults from clinicians for participants.
• Gathers information from hospitals, health plans, physicians, patients, and other sources.
• Maintains focus while multi-tasking calls, texts, and electronic queues.
• Supports a hybrid/virtual work model with in-person training when required.
