Maven Clinic is the world’s largest virtual clinic for women and families, offering 24/7/365 care and a broad set of programs. We’re seeking a mission-driven Health Coach to coordinate care for Maven members, drive engagement, and support high-risk individuals through a data-informed coaching model.

You’ll work within a remote Care Delivery team, collaborating with providers and leveraging digital tools to improve outcomes and ensure timely care.

Competenze

• 1-2+ years experience in health coaching or clinical work with focus on patient education and outcomes.
• Experience with maternity care or family planning navigation.
• Coaching to achieve specific health outcomes (medication, nutrition, exercise).
• Flexible, growth mindset, adaptable to demand.
• Empathetic written and verbal communication for patients.

• Drive program engagement and communicate value of care coaching model.
• Co-create SMART goals and track progress with members.
• Manage caseload via messaging, video appointments, and outreach.
• Follow clinical guidelines and use motivational interviewing to promote change.
