Comagine Health in New Mexico is seeking a Home and Community-Based Services Assessor to conduct annual in-home assessments for adults enrolled in the Mi Via Waiver across SE NM. The role combines field visits with a home-office for follow-up, scheduling, and documentation.

You’ll use structured assessment tools, ensure HIPAA compliance, and coordinate with a growing team. A driver’s license and reliable transportation are required, and residing in Southeast New Mexico is needed.

Qualifications

• Associates degree in social work, psychology, or other behavioral health field.
• 2 years of social service or related work experience.
• Valid, unrestricted driver's license and reliable transportation.
• Ability to pass Motor Vehicle Record check.

Responsabilités

• Conduct annual in-home assessments across the southeast New Mexico region.
• Visiting participants’ homes and documenting daily living skills, behaviors, and support needs.
• Use electronic assessment tools and multiple data systems to document findings and close cases timely.
• Maintain confidentiality and HIPAA compliance; communicate with supervisor and team.
• Participate in trainings and team meetings.
